Are we in complete control of spontaneous VF? Despite a 10-joule safety margin and low defibrillation thresholds (DFT) of less than 15 joules at testing during implantation, first shock success rates for spontaneous rapid arrhythmias are only 83% to 93%.1,2,3 Why does an induced rapid arrythmia differ from a spontanous one? Do you know how high your patient’s DFT is today or will be tomorrow? Multiple studies have identified many factors that are correlates of higher DFTs. Cumulatively, these factors might decrease defibrillation efficacy. Failed ATP and long episodes Antitachycardia pacing (ATP) is widely used as a first therapeutic approach in clinical practice.
