Cascade DAFO Inc 1360 Sunset Avenue Ferndale, WA 98248 phone: 1-800-848-7332 fax: 1-877-856-2160 website: www.dafo.com DAFO 5 Maximum Control Foot Orthosis UCB TYPE CUSTOM MOLDED SHELL Hindfoot: maximum control possible in low trimline, low to normal tone. Forefoot: maximum control possible in low trimline. Ankle: minimum control. This 5 model is shown with forefoot strap, medial padding and toe rise. Formed on a custom mold of the patient’s foot, the DAFO 5 provides good foot control for a hypotonic patient who is beginning to use voluntary graded muscle motion. Trimlines are brought high to avoid edge pressure. Enclosed design provides good forefoot control when working in supportive shoe. Submalleolar hindfoot trimline limits the use of DAFO 5 to low-to-normal tone feet. USE WITH PATIENTS WHO: 1. Collapse into pronation during midstance (e.g. Down’s Syndrome). 2. Exhibit excessive foot mobility but are fully correctable 3. Have good dorsiflexion range in corrected position 4. Need foot surrounded more than is possible with lower trimline shoe inserts 5. Pronation is due to low tone, delayed development, poor proprioception.
