Streamline Communications with Physicians and Payers Collaborator Secure Web Portal for Home Health and Hospice Agencies From sharing patient information and collecting signatures on orders to reviewing authorization requests, physicians and payers play a key role in the workflow of home health and hospice agencies. Traditionally, the patient orders and clinical information captured in a patient’s electronic medical record (EMR) are sent in a paper form from the agency to the physician or payer through mail or fax. That’s why Cerner developed Collaborator, a secure web portal designed to help agencies get authorizations and documents signed more quickly, leading to faster reimbursements and improved patient care. With Collaborator, patient information is sent electronically from Cerner’s Home Care enterprise solution HomeWorks to the internet portal, where the physician or payer can log in and manage their tasks immediately. Orders and attestations signed in the portal can be received directly back into HomeWorks, updating all records for rapid turnaround. The internet portal facilitates increased collaboration in caring for patients between agencies, physicians and insurance case managers. Increase Patient Care Streamlined communications and collaboration using the web portal leads to faster, quality care for patients. Physicians can electronically view patient electronic medical records, providing them a comprehensive view of their patients’ status. A list of patients is displayed upon login, enabling physicians to quickly review patient charts and sign pending orders. Insurance case managers using the portal can review patient charts to determine eligibility for authorization of services. Simplify the Face-to-Face Increased turnaround times for the face-to-face 30-day requirement can be achieved using the web portal, ensuring full credit is received for the care being provided to patients. With a simple login, physicians can review all pending face-to-face attestations for patients, and electronically sign them with the click of a button. Physicians can see the dates and times they have available for face-to-face encounters. Strengthen Relationships Increase the physician experience with the portal and increase referrals.
