Test Principle: The PMR test uses a pH indicator, nitrazine yellow, to detect the difference between normal vaginal pH (4.5-6.0) and amniotic fluid pH (7.0-7.5). A color change indicates the presence of amniotic fluid, distinguishing it from cervical and vaginal mucus or urine.
Procedure:
  1. Clean the vulva and vaginal orifice.
  2. Insert a speculum to expose the cervix.
  3. Remove the swab from packaging without contamination.
  4. Contact the swab with liquid at the cervical os or posterior fornix for 5 seconds.
  5. Withdraw the swab and observe the color change within 30 seconds.
  6. Compare the color with the provided chart.
Interpretation:
  • Intact membranes: pH 5.0-6.0 (yellow)
  • Ruptured membranes: pH 6.5-7.5 (blue-green to blue-black)

Principle PMR tests are individually wrapped sterile swabs impregnated with a sensitive pH indicator, nitrazine yellow. The principle of the PMR test is based on the detection of the difference between the normal vaginal pH, at pregnancy term (4.5-6.0) and the pH of amniotic fluid (usually 7.0-7.5). By means of a simple pH colour change of nitrazine yellow, which has a sharp end point (6.4-6.8), exactly in the desired pH range, the obstetrician can accurately detect the presence of amniotic fluid.
