Acetabular Preparation: After exposing the joint and performing a femoral neck osteotomy, the acetabulum is reamed to bleeding bone using progressively larger hemispherical reamers, ensuring the planned orientation is respected. Acetabular Trial: A trial cup matching the last reamer size is selected, attached to the trial shell handle, and impacted into the reamed acetabulum at a 45° inclination. The trial cup should have good primary stability and fit tightly. Stability is assessed by spinning and rocking the trial before impacting the definitive cup.
Shell Positioning: The definitive cup size should match the trial cup used for stability assessment. The PAVI cup is impacted using a handle with a shell holder, respecting the validated anteversion and inclination. Additional fixation with screws is possible using specific instruments.
Liner Insertion: For ceramic liners, position manually parallel to the shell face and slightly impact. Ensure perfect alignment. For hooded polyethylene liners, use an impaction plate and replicate the trial liner's orientation.
Preoperative Planning: X-ray templates are used to determine the appropriate shell size and optimal implant position in the acetabulum.
