Introduction
The modified Ventral Hernia Working Group (VHWG) classification system, developed in 2012, aims to predict surgical site occurrences (SSO) after open ventral hernia repair. SSOs include infections, seromas, wound dehiscence, or enterocutaneous fistula formation.
Hernia Type and Mesh Type
The classification considers different types of mesh materials: Permanent Synthetic, Resorbable Synthetic (Biosynthetic), and Biologic.
VHWG Classification
  • Grade 1: Choice of repair material depends on surgeon and patient factors.
  • Grade 2: Increased SSO risk suggests a potential advantage for biologic reinforcement over permanent synthetic materials.
  • Grade 3: Permanent synthetic materials are generally not recommended; biologic repair materials may be advantageous.
  • Grade 4: Permanent synthetic materials are not recommended; biologic repair materials should be considered.
Risk Levels and Characteristics
  • Low Risk: Low complication risk, no history of wound infection.
  • Co-Morbid: Includes smokers, obese, diabetic, immunosuppressed, or COPD patients.
  • Potentially Contaminated: Previous wound infection, stoma presence, or gastrointestinal tract violation.
  • Infected: Infected mesh or septic dehiscence.
Modified VHWG Classification and Risk
  • Grade 1: Low risk with 14% SSO incidence.
  • Grade 2: Co-morbid conditions with 27% SSO incidence.
  • Grade 3: Contaminated conditions with 46% SSO incidence, further categorized into Clean-Contaminated, Contaminated, and Dirty.
