Product Overview
The PFF Modular Hook system is designed for proximal femur fractures, offering stability features from the O’nil system with an optional modular GT hook for additional stabilization of the greater trochanter. It incorporates a patented conical locking system that provides angular stability, especially beneficial in osteoporotic bone where screw fixation may be compromised. The screws act as fixators rather than relying on plate compression, preserving periosteal blood supply.
Two proximal screws (Ø7mm) diverge by 8° to facilitate insertion and follow femoral neck anteversion, ensuring stable fixation in the trochanteric region. The modular GT hook anchors at the apex of the greater trochanter. The anatomical plate profile supports metaphyseal to diaphyseal reduction and helps restore the neck-shaft angle.
Indications
- Proximal femur fractures including simple trochanteric, cervicotrochanteric, trochanteric-diaphyseal, multifragmentary pertrochanteric, intertrochanteric (including reverse or transverse), and fractures with medial cortical involvement.
- Proximal femur fractures combined with ipsilateral diaphyseal fractures.
- Fractures with proximal femur metastases.
- Proximal femur osteotomies.
- Fixation in osteoporotic bone and treatment of nonunions or defective consolidations.

Usage Procedure
- Position the PFF plate and insert drill guides, reducers, and 2.5mm guide wires into screw holes A and B.
- Measure screw length without removing drill guides.
- Drill a 5-6 cm hole with a 5.0mm cannulated drill.
- Remove drill guides and insert Ø7mm screws using a hex screwdriver.
- Repeat for both proximal screw holes.
- Use 4.0mm drill and depth gauge for diaphyseal screws, then insert screws accordingly.
- If using the modular GT hook, remove adjustment pin, position the hook with the GT PFF positioner, and reinsert the adjustment pin.
- Complete implant fixation.
Ordering Information
Plates are available in multiple lengths (from 12mm to 110mm epiphysis holes and 60mm to 120mm diaphysis holes) with specific references for right and left versions. Screws are available in Ø5.0mm and Ø7.0mm self-locking variants. Materials conform to ISO standards: plates in AISI 316LVM stainless steel (ISO 5832-1) and screws/washers in titanium Ti6Al4V (ISO 5832-3).
Instrument Set
A dedicated instrument set (Series 5) supports the PFF system, including drill guides, cannulated drills, hex screwdrivers, depth gauges, K-wires, and specialized tools for modular GT hook placement and screw extraction.
