Pernicious Anemia and Vitamin B12
Pernicious anemia, also known as Biermer's disease, results from a vitamin B12 deficiency often due to a lack of intrinsic factor. Vitamin B12 is crucial for blood cell formation and is absorbed in the small intestine with the help of intrinsic factor, a glycoprotein produced by gastric parietal cells. A deficiency in intrinsic factor leads to reduced vitamin B12 absorption and subsequent anemia.
Pathogenesis
Type A gastritis is a chronic autoimmune condition where antibodies target parietal cells and intrinsic factor, impairing vitamin B12 absorption. Other causes include genetic factors, surgical removal of stomach sections, chronic gastric inflammation, certain medications, and unbalanced diets.
Clinical Symptoms
Symptoms of pernicious anemia include fatigue, increased heart rate, paleness, indigestion, abdominal pain, and neurological issues such as numbness, unsteady gait, and coordination disorders. Severe cases may lead to funicular myelosis.
Diagnosis
Diagnosis involves clinical symptom assessment and laboratory tests for antibodies against parietal cells, H+/K+ ATPase, and intrinsic factor.
Assay Details
The assay uses recombinant human intrinsic factor and is calibrated with an internal reference sample. Results are expressed in U/mL. The assay demonstrates high precision with intra- and interassay variations and excellent diagnostic sensitivity (89%) and specificity (>99%).
