Positioning:
The procedure is conducted under general anesthesia with the patient in a supine position. This positioning facilitates lateral imaging of the cervical vertebrae by pulling down and securing the patient's shoulders.
Corpectomy and Endplate Preparation:
A partial or complete corpectomy is performed based on the pathology. Disc material and superficial cartilaginous layers of adjacent endplates are excised. Proper cleaning of the endplate, especially peripherally, is crucial for vascular supply.
Implant Size Determination:
The intervertebral distance is measured to select the implant height. The implant's neutral height should be less than the defect height, while its expanded height should exceed the defect height, including anchorage.
Implant Preparation:
The mesh is trimmed to the appropriate height using a mesh cutter. Standard rings are placed inside the mesh at the desired location.
Device Insertion and Placement:
Before implantation, a holder is used. The expandable corpectomy cage is inserted and aligned in the sagittal and frontal planes, ideally centered on the vertebral endplate. Space around the implant endplates is maintained for bony fusion.
Applying Bone Material:
The area near vascularized tissue is filled with the maximum possible bone graft, particularly in the anterior part of the instrumented zone, to promote fusion and stability.
Supplemental Fixation:
For spinal stability and compression maintenance, expandable corpectomy cages require supplemental fixation. An anterior plate is screwed in to immobilize the spine.
