Step 2 Once the PO and patient form has been received, this will generate an order for a 30 day trial period for the KIT-PLIEMPK. You will not be charged or invoiced at this stage. Step 3 Following a successful trial, call Customer Services on 0870 240 4133 to obtain an RTA number and return the demo Plie 3 knee only. If the trial was unsuccessful, call Customer Services on 0870 240 4133 to obtain an RTA number and return the full kit. Step 4 - Successful Trial Only After contacting Customer Services your order will then be finalised. The definitive Plie 3 knee and the preferred designer fairing stated on this form will be delivered and invoiced. Adapters The following adapters are included within the MPK package.
