Plie 3 MPK Package - Order Form included in the package: • Plie 3 knee, including 5 year warranty & the following items: • Wireless USB adapter • Lithium-ion battery case • Software CD • Lithium-ion battery x2 • Battery charger with car adapter • Air pump with hose adapter • Smooth hose adapter for air pump Preferred designer fairing Choice of foot - Kinterra, Sierra, Highlander, Maverick Xtreme, Maverick Xtreme AT or Maverick Comfort AT Set of adapters Step 1 Submit a PO using part number: KIT-PLIEMPK and your completed order form. Step 2 Once the PO and patient form has been received, this will generate an order for a 30 day trial period for the KIT-PLIEMPK. You will not be charged or invoiced at this stage. Step 3 Following a successful trial, call Customer Services on 0870 240 4133 to obtain an RTA number and return the demo Plie 3 knee only. If the trial was unsuccessful, call Customer Services on 0870 240 4133 to obtain an RTA number and return the full kit. Step 4 - Successful Trial Only After contacting Customer Services your order will then be finalised. The definitive Plie 3 knee and the preferred designer fairing stated on this form will be delivered and invoiced. Adapters The following adapters are included within the MPK package. For alternative adapters, please place a separate order. Set 1 Designer Fairings Please select the preferred designer fairing. This will be supplied on the definitive Plie 3 knee upon completion of a successful evaluation and will not be shipped with the demonstration unit. High: Daily activities include fast walking, jogging, running, lifting heavy objects and/or recreational sports. 1 Sierra, Highlander, Maverick Xtreme, Maverick Xtreme AT & Maverick Comfort AT only. 2Sandal toe available in sizes 22-28 for Kinterra, Sierra & Highlander. Sandal toe available in sizes 23-28 for all Maverick feet. The extended warranty on the Plie 3 from 3 to 5 years is automatically included within the MPK package. Please note, the Plie 3 knee must be returned for annual servicing to maintain the warranty. A loaner unit will be provided during this time.
