TWO WEEK TRIAL GUIDELINES Thank you for your interest in our products. In order to fully appreciate the value of the PEDIASCAN® or MAXISCAN® transilluminators, we are offering customers a TWO WEEK “no obligation” trial period. This trial gives the end user the opportunity to see how our product performs in your department. Please see below the guidelines for setting up a trial: 1.) All trials must have a “no charge” or “evaluation” official purchase order (P.O.) number assigned to them by the ordering hospital’s purchasing department. No exceptions will be made. 2.) All trial P.O.’s must indicate the director of purchasing, the person ordering and the end user of the product ordered along with their phone numbers and email addresses 3.) All trial orders must be either faxed, emailed or mailed to Sylvan Corporation. Verbal orders will not be accepted. 4.) All trials are effective for TWO WEEKS unless extended. Just call us and we can give you more time if you need it. We reserve the right to invoice you as a monthly rental if the unit is not returned in accordance with this agreement. Contact us for monthly rental fees for the unit that you are trialing. 5.) All units sent out on trial are brand new and can be purchased by changing the P.O. The warranty will start as of the date of the new P.O. 6.)
