Journal of Clinical Medicine in Practice Effect of total parenteral nutrition infusion sets in prevention of phlebitis caused by nutrition infusion CHEN Miao-xia (The Third Hospital Affiliated to Sun Yat-sen University, Guangzhou, Guangdong, 510630) Abstract: Objective To evaluate the effect of total parenteral nutrition (TPN) infusion sets to prevent phlebitis caused by nutrition infusion. Methods A total of 156 patients were divided into experiment group (n=80) and control group (n=76). The patients in the experiment group used TPN infusion sets with 3μm precision filter, and the patients in the control group employed common disposable infusion sets. Then statistical analysis to the proportions of phlebitis was conducted. Results 36 patients (47.37%) in the control group and 7 patients (8.75%) in the experiment group suffered from phlebitis. The incidence rate of phlebitis in the experiment group was significantly lower than in the control group (P<0.01). The outbreak of phlebitis in experiment group was significantly slower than in the control group. Conclusion Using TPN infusion sets has significant effect on the prevention of phlebitis induced by nutrition infusion. Key Words: TPN; infusion sets; phlebitis Parenteral nutrition (PN) is feeding a person intravenously, for those who cannot receive enough nutrition by the usual process of eating and digestion. It contains nutrients such as amino acids, lipids, glucose, vitamins and dietary minerals to provide patients the necessary materials to recover or grow[1]. It is called total parenteral nutrition (TPN) when no nutrition is obtained by other routes. But precipitation could develop in the TPN admixture due to the instability of lipid emulsion, calcium and phosphate. After the report of 2 deaths and several cases of respiratory distress caused by the precipitation in TPN, FDA suggest that a filter should be used in TPN infusion[2]. Currently, the pore size of filtering membrane in commonly used precision infusion sets is big, with unsatisfactory filtering property. Meanwhile the long-time soaking of cellulose membrane of common infusion sets in drug liquids will expand and distort the membrane, which lead to the descrease of filtration precision. If TPN without passing the filter is infused into human body, the insoluble particles within the solution will cause severe harm to the patient. In order to study the effect of different infusion sets on TPN infusion, we will use disposable TPN infusion sets and commonly used infusion sets to infuse TPN, to observe the incidence rate of phlebitis and outbreak of phlebitis under the use of two types of infusion sets. Objects & Methods During January ~ May 2011, a total of 156 patients received TPN therapy in our hospital. Among which there were 83 males, 73 females, age in the range of 20~76 years old with the average of (52.3±4.9) years old. These patients were randomly divided into the experiment group (80 cases) and control group (76 cases). There was no significant difference in gender proportion, age composition, condition and therapy between the two groups, and they are comparable.
